One Seriously Injured in Head On Crash on State Route 121 in Darke County

DARKE COUNTY — One person was seriously injured and two others were hurt in a two vehicle head on crash Wednesday afternoon on State Route 121, according to the Darke County Sheriff’s Office.

The crash happened at about 4:23 p.m. in the 7300 block of State Route 121. Deputies responded along with Greenville Township Fire and Rescue and Greenville City Fire.

According to the sheriff’s office, a silver Chevy Equinox driven by 70 year old Anthony Mader of Bradford was traveling southbound with 47 year old Lori Perkins of Greenville in the front passenger seat when the vehicle crossed the centerline and struck a maroon and silver Ford dually pickup head on.

Investigators say the pickup was driven by 19 year old Christian Madowell of Greenville. After the impact, the truck went off the right side of the road and overturned twice. The sheriff’s office says the cab broke off the frame and came to rest upside down, trapping the driver inside.

Madowell was mechanically extricated from the cab and flown by MedFlight to Miami Valley Hospital with suspected serious injuries.

Mader and Perkins were both taken to Wayne HealthCare with suspected minor injuries.

The sheriff’s office says investigators believe the driver of the Equinox may have suffered a medical event that caused the vehicle to go left of center.

The Darke County Sheriff’s Office Accident Reconstruction Team assisted in the investigation.
